Please fill out the form with as much information as possible. Failure to be 100% honest will result in the ban appeal being revoked. If you have any evidence that proves your innocence please add it on the form.
Do not be impatient and wait until a server admin has reviewed your appeal, you will be contacted when a decision has been made.
